What is Liberate Pro?
Liberate Pro is a unique educational tool, designed for health care professionals to maximise the impact of them to improve health outcomes.
The app enables clinicians to engage patients in a stimulating and interactive way during a consultation. It provides access to accurate, understandable information and visuals that patients need to best manage their condition and comply with their treatment.
Clinicians can effortlessly customise pre-loaded condition-specific content, adding notes, attachments and annotating visuals to make it specific to the patient’s individual case.
Piloting Liberate Pro with Suffolk and North East Essex Integrated Care System
Health Innovation East has worked closely with Suffolk and North East Essex Integrated Care System (SNEE ICS) to broker a pilot of this tool with cardiovascular disease patients, with the evaluation due in January 2024. Health Innovation East has supported the development of resources for the pathway and materials aimed at both clinicians and patients. This is being further extended to support other areas including pulmonary and cardiac rehabilitation.
All content within Liberate Pro has been developed by medical thought leaders and health literacy experts to ensure it is presented in a way patient understand. Clinicians also have the option to record relevant parts of the consultation which they want to ensure are remembered and understood.
The personalised content and consultation can then be sent to a secure patient portal (by the clinician), so that the patient can playback, read and digest what they were told and share it with family or carers.
Providing patients with accurate and understandable information empowers them to take greater control of their health. The Liberate Health multi-platform app, enables clinicians to engage patients in a stimulating and interactive way during a consultation. We have entered into a revenue share with Liberate Pro and Health Innovation East is supporting further development of the app and is looking to pilot the product in the NHS.
