Assessing anticipatory care in SNEE

Health Innovation East has been commissioned by SNEE ICS to conduct an independent baseline review of current readiness within primary care to deliver anticipatory care by April 2023. It is hoped that this can support more integrated care delivery across the ICS.

Helping to shape how anticipatory care is delivered in primary care across SNEE

Health Innovation East is working with SNEE ICS to conduct a baseline review of current readiness within primary care to deliver anticipatory care by April 2023. This baseline will help to inform the development of an ICS-wide strategic action plan.

We are conducting short interviews with staff within PCNs from Ipswich and East Suffolk Alliance and North East Essex Alliance until mid-August to understand their current activity and priorities for development.

What is anticipatory care?

Anticipatory care is proactive healthcare and support, targeted at people of all ages living with frailty, multiple long-term conditions and/or complex needs to help them stay independent and healthy for as long as possible at home. It focuses on providing the support that is most important to them to reduce the risks of their health worsening.
